For the assessment of medical fitness to fly you can submit your documents using the digital form on our website. We process the information you enter (last name, first name, date of birth, address, phone number, e-mail address, AHV number, place of birth or origin, medical history, medication and allergies), any additional documents (e.g. operation reports or prescription values) and your digital signature. This is particularly sensitive personal data.

The form is compiled into a PDF in your browser and transmitted encrypted (HTTPS) to our web server. The web server does not store this data; it forwards it exclusively by e-mail to the medical practice. The documents are added to your patient file and retained in line with statutory retention obligations. The purpose of processing is the preparation and performance of the aviation medical examination. The legal basis is your explicit consent, which you give before submitting. Disclosure to the FOCA, an aeromedical center or examiner, or other bodies only takes place within the scope of the declaration of consent you sign.

For confirmation we send you an e-mail to the address you provide, with a copy of your completed and signed application attached as a PDF. Please note that e-mails are not always transmitted with end-to-end encryption.
